there is a little window with dlc in the your library in the binding og isaac rebirth, you need to check the little box with afterbirth on it

This is not a full game, is a DLC, so will be inside TBOI: Rebirth as a DLC and this game will have a lot of new items. Check again in Properties/DLC on TBOI: Rebirth Game-
