Story-Related
I think descriptions are unnecessary, as they are obtained during the storyline progression.

Obey Me
Dominate an Uruk.

Sometimes (probably if there's a lag with Steam connection) it doesn't unlock. Creating a new game doesn't help.
You need to rename the file `gamedata.sav` in the root folder "...Steam/userdata/*your ID*/241930/remote" (and disable the cloud).
When you launch the game, new saves will be created, and soon the achievement will unlock.
Then go back to the root folder, delete the new `gamedata` document, rename your old saves back to their original name, and enable cloud synchronization.

The Bright Master
Learn about the Wraith's past.

One Ring to Bind Them
Witness the creation of the Rings of Power.

Ratbag the Great and Powerful
Help Ratbag become a Warchief.

Gorthaur the Cruel
Destroy the monument gates and topple Sauron's statue.

The Hammer Falls
Kill The Hammer.

Brothers in Arms
Kill the great white Graug.

Lord and Master
Brand all five Warchiefs.

Of course, this can be done outside the story, but since it's required by the story, I've included it here.

The White Wizard
Free Lady Marwen from her curse.

No Power in Numbers
Complete the quest chain for Marwen's daughter.

The Tower Crumbles
Kill The Tower.

The Hand that Severed
Kill The Black Hand.
Unlocks

Memories of Eregion
Activate all Forge Towers.

There are 12 towers in the game:
6 in Udûn and
6 in the Sea of Nurnen
The towers are marked on the map. You need to climb them and strike the anvil with a hammer.

Strike True
Unlock 2 Rune slots on the Bow.

The Cold Light
Unlock 3 Rune slots on the Sword.

Paid in Blood
Unlock 4 Rune slots on the Dagger.

To unlock a Rune slot, you will need Mirian. You earn it by completing side missions, finding artifacts, Ithildin, etc.
Missions
Sword Legends.

Legend of Vengeance
Start creating the legend of the sword Urfael by completing one of its missions.

Flame of Justice
Complete all "Legend of Urfael" missions.

10 missions where you will fight large groups of Uruks, creating legends about the sword.

Bow Legends.

Legend of the Maker
Start creating the legend of the bow Azkâr by completing one of its missions.

Maker's Bow
Complete all "Legend of Azkâr" missions.

10 missions where you will fight Uruks with a bow at close and long range.

Dagger Legends.

Legend of the Shadow
Start creating the legend of the dagger Acharn by completing one of its missions.

The Last Shadow
Complete all "Legend of Acharn" missions.

10 missions where you will stealthily take down Uruks. Like the bow missions, these require some skill.

Outcast Rescue Missions.

The Free Folk
Complete an Outcast Rescue mission.

Liberator
Complete all Outcast Rescue missions.

24 missions to rescue slaves. Note that for these missions to appear, you need to free slaves marked with an unframed fist icon, which appear randomly when there are slaves nearby. To avoid running around the entire map looking for slaves, move from tower to tower and check the mini-map. Once you free the slaves, missions will appear on the map.

Vengeance is Mine.

Blood for Blood
Complete a Vendetta mission.

Missions will periodically appear on the map where you need to avenge a fallen friend. If your friends don't have this game, you'll have to avenge a random player.

After the servers are shut down:
On the developer's Discord channel, it was stated that the "Blood for Blood" trophy now unlocks through personal vendettas when "you are killed by a captain and then avenge yourself." The Vendetta captain you need to kill for the trophy will be marked on the army screen.
Survival Challenge

Scout of the Morannon
Successfully complete a Survival Challenge.

Ranger of Ithilien
Complete all Survival Challenges.

There are a total of 10 challenges. To complete them, you need to collect various plants and herbs marked on the mini-map. In the upper left corner of the map, under the "Survival Challenge" label, the herbs you need are displayed. Additionally, when hovering over an area on the map, it shows which herbs grow there.
Hunting Challenges

The Thrill of the Hunt
Successfully complete 4 Hunting Challenges.

Master of the Wilds
Complete all Hunting Challenges.
In these challenges, we will hunt almost all of Mordor's inhabitants.

I'll attach a video, but be sure to read the tips below for useful advice.

  • Challenge 1: Kill 3 spiders.
Spiders roam all over the map, so finding them shouldn't be too difficult. If you're having trouble, use Wraith mode.

  • Challenge 2: Kill 5 Hell-hawks.
Look up at the sky. See the birds flying? That's them! If you don't see any, return to the area where the game started. They're definitely there.

  • Challenge 3: Kill 3 Caragors.
You can find Caragors in 3 ways: in cages (in Uruk camps), by bait (a piece of meat hanging in some camps), or just by roaming the map. They can be killed with two aimed shots to the head.

  • Challenge 4: Kill 4 Mordor Rats.
Look for them near Uruk camps. Use Wraith mode.

  • Challenge 5: Kill 8 Ghûls.
    Hunt Ghûls at night!
They're easiest to find in caves, but they also appear on the map. They always appear in groups but die from one hit.

  • Challenge 6: Kill 3 Morgul Bats.
These bats can be found near cave entrances. It's better to kill them from a distance; otherwise, they'll scatter when you get close.

  • Challenge 7: Kill a Graug.
Graugs are the largest and most dangerous creatures in Mordor and can easily kill you if you're not careful. Attack Graugs while they're stunned (two aimed shots to the head). But it's easiest to kill a Graug after acquiring the "Shadow Mount" ability, allowing you to dominate and then kill it with a few button presses.

    Tips:
Complete Torvin's quest to kill a Graug. Once you've killed a regular Graug, don't continue Torvin's quests.

  • Challenge 8: Kill 1 Dire Caragor.
The Dire Caragor has a white color, spikes, and a health bar similar to a Graug. It's most commonly found in the Sea of Nurnen.

  • Challenge 9: Kill 1 Ghûl Matron.
A Ghûl Matron is larger than a regular Ghûl and as strong as captains. To find a Ghûl Matron, locate regular Ghûls and kill them, leaving only a few alive. They'll run for reinforcements. If the Matron doesn't appear, repeat until it does.
The Ghûl Matron only appears at night.

    Tips:
It's best to complete this during the Legend of Urfael mission. In one mission, you'll need to kill 3 groups of Ghûls, after which the Ghûl Matron will appear.

  • Challenge 10: Kill 1 Horned Graug.
Similar to a regular Graug but stronger and with horns. It can be found in the Sea of Nurnen in a cave on the left side of the map and in Udûn in the same area (see screenshot). If it's not there, advance time using a Forge Tower and return (repeat until it appears).
Ithildin
These are mysterious markings carved on the walls of ancient ruins.

Shadows of the Ancient Past
Collect 50% of the Ithildin.

Bearer of the Bright Star
Collect 100% of the Ithildin.

Ithildin is marked on the map, making it easy to find.

Notes:
  • Artifacts can only be seen in Wraith mode.
  • Listening to a memory means finding the blue point by rotating the item.
  • If you haven't unlocked this achievement, it's solely due to your oversight! There are no technical issues; just go through again and find what you missed.
Captains and Warchiefs
Do not confuse Warchiefs with Captains! Warchiefs are in the top row, while Captains are everything below! Additionally, to make a Warchief appear, you need to complete a mission. Captains, on the other hand, can be found on the map or by going to a "Hunt" mission, etc.

The Graug's Foot
Exploit a Warchief's fear.

Discover a WARCHIEF's fears, such as a fear of fire, and when the Warchief is near a bonfire, blow it up. When the Warchief starts running, chase him down and kill him.

New Master
Brand a Captain during combat.

Deliver a series of strikes until the combo meter turns red, then press the required button combination (A+B on an Xbox controller). Make sure the skull under the health bar is green; otherwise, there's a high chance of failure.

The Spirit of Mordor
Start a riot by commanding a Warchief to attack another Warchief.

Brand a Warchief and command him to attack another.

A Feast to Remember
Poison a Captain at his own feast.

Find a "Feast" mission. If it's not available, advance time until it appears, and during the mission, poison the grog barrel. Wait for the Captain to approach and drink.

Rise and Fall
When an Uruk kills you and becomes a Captain, help him become a Warchief, then kill him.

Die at the hands of a regular Uruk, brand him, and promote him to Warchief. Then, kill him with a single button press.

Stinking Rebels
Brand 5 bodyguards of a Warchief and have them turn against him.

Warchiefs usually have 2-3 bodyguards. Brand their bodyguards, then send your own to make up the difference to 5 (if needed, brand Captains). Challenge the Warchief to a fight and command the bodyguards to attack him during the battle.

Divide and Conquer
Kill the bodyguards of two Warchiefs, then draw out and kill both Warchiefs.

First, kill the bodyguards, then kill the Warchiefs one by one (not simultaneously).

Power Vacuum
Kill all five Warchiefs before any Uruk takes their place.

This is essentially a story achievement. It will be completed during Ratbag's missions. Don't worry about dying; no one will take the Warchiefs' places until you complete these quests.
There's an easier way to complete the "Power Vacuum" achievement for those who didn't get it with Ratbag's quests. After branding all the Warchiefs (as part of the story), summon them one by one (blue icon on the map with a framed skull, indicating the Warchief is ready to follow your orders). Once the Warchief arrives, kill him with a single button press, and repeat for each one. This won't take much time, about 3-4 minutes.

Iron of Death
Issue a death threat, and then kill the target.

First, upgrade this skill in the menu. Then find a Worm, Captain, or Warchief and break their will, choosing "Threaten" instead of "Gather Intel." Note that if you have already gathered intel on your target, you cannot threaten them.

No Power in Numbers
Help a Captain in a recruitment power struggle, then kill him and all his new recruits.

Look for a recruitment mission. If it's not available, advance time. It doesn't matter if you help a branded Uruk or not; the main thing is for the Captain to survive at the end! Once the mission is complete, eliminate the Captain.
Caragor

Unleashed
Free 5 Caragors from cages.

Shoot the cages with Caragors using your bow.

Jaws of Death
Attract Caragors 5 times using bait.

Meat carcasses hanging from walls. Most commonly found in fortresses (in large quantities).

The White Rider
Free 30 slaves in 180 seconds while riding a Caragor.

Mount a Caragor and ride around the map looking for slaves (marked by small black arrows). When you find them, kill the Uruks near them and continue searching. Repeat until you unlock the achievement; it won't take much time.
Miscellaneous

Fly, You Fools!
Make 20 Uruks flee by dropping Morgai Fly nests.

The easiest way: gather a crowd in a fortress and shoot the hive.

Burning Vengeance
Burn 50 Uruks using the "Detonate" ability.

See Uruks near a bonfire or barrels? Shoot them!

Height of Despair
Perform an aerial takedown from at least 60 feet above the target.

Look for a tall structure and leap onto an Uruk below. To estimate the height, I completed it by jumping from a Forge Tower.

Fire of Justice
Execute a burning Berserker.

Of course, you can ignite with a fire arrow or dart (if you have a rune for it), but I often had them die in the bonfire, so it took some effort.

The Power of Fate
Obtain a level 25 rune.

Easily achieved in the Bright Lord DLC! But remember, if you kill, a legendary rune might drop, so let branded Uruks do the killing.

The rune level consists of the following factors:

1. Fulfill all conditions except the death threat, as it increases the chance of a legendary rune drop, which we don't want.

You might be lucky and get the desired rune, but most likely it will be a legendary one! Therefore, the second and most important tip: when everything is ready and only the attack on the Warchief remains, exit the game and back up your saves (located here: ..\Steam\userdata\78066698\241930) so you won't have to level up a new Uruk constantly. Just replace the saves with the old ones and try again.

P.S. ▲ The Uruk who killed you becomes the target of vengeance.

2. 100% method!
Create a Warchief as noted above (not branded). You'll also need one branded Warchief and command him to attack the first. The trick is to have your Warchief kill the other, and then you'll get the desired rune.

DLC The Great Hunt
Note that 4 achievements are best completed during missions!
  • Flames of Vengeance
  • Mumakil, Where Are You?
  • Ghûls Gone Wild
  • Collector

Short Introduction
Start a new hunt with Torvin.

Complete the first mission.

Jaws of Shadow
Perform 25 stealth kills while mounted on a Caragath.

Execute stealth kills as usual, but while mounted on a Caragath.

Had to Put Him Down
Execute a Caragath.

While mounted on a Caragath, hold RT + B (L.Shift + E).

Flames of Vengeance
Set a Ghûl Matron on fire during "The Great Hunt."

During the mission where you need to shoot Ghûl nests, wait until the meter turns red and shoot the Matron with a fire arrow.

Stirred the Hive
Lure Ghûls from 10 mounds.

After learning to shoot mounds, find them on the map and shoot them.

Vile Bile
Have a Wretched Graug vomit on a Captain or Warchief.

Mount a Graug and find a Captain. Approach, aim, and vomit on him. While he's writhing, press Y to eat him and earn the "Nom Nom Nom!" achievement.

Nom Nom Nom!
Have a Wretched Graug eat a Captain.

Find a Captain, and when near him, press Y to eat him.

Mumakil, Where Are You?
Brand 20 Ghûls at once.

Can be done during the mission to learn Ghûl branding.

Ghûls Gone Wild
Kill a Graug by dispatching Ghûls on it.

During the Ghûl branding mission, a Graug will appear. When it's near death, detonate the Ghûls. If unsuccessful, just move away from the mission and repeat from the Graug's attack.

Burning Steed
Ride a blazing Caragath.

After completing all hunter missions, you'll gain the ability to set a Caragath on fire.

Collector
Simultaneously control a Caragath, Wretched Graug, and Spitting Ghûl.

Can be achieved during the battle with the final Warchief. When the Warchief exits the cave, he'll wait for you near the ruins. Bait for a Graug and Caragath hangs on these ruins (he summons the Ghûls himself). All you need to do is take control of them!

OR

The Most Dangerous Game
Defeat all Beastmaster Warchiefs.
DLC Test of the Wild
In the game, go to "Trials of War" and select "Test of the Wild."

Hunting - My Lady Love
Complete all objectives in the Test of the Wild.

In this challenge, you need to:
  • Kill 5 Warchiefs and 20 Captains.
  • Kill 10 Captains using beasts.
  • Score 13,000 points.
  • Complete the challenge in under 40 minutes (+1000 points).

Tips:
  • Ignore dropped runes! You can collect them later in the campaign.
  • Don't focus on points! You'll likely accumulate them as you progress through the challenge. If you don't, you'll have time to correct it.
  • If you have to travel on foot, do it on a Caragor—it slows down time (jump over an obstacle and press the prompted button).
  • In the upper right fortress, you can only enter the part after the bridge on a Graug; it won't fit further. So you'll have to kill Captains inside the fortress manually. Of course, there are Caragors, but they usually get killed unless there's a fear of Caragors.
  • If one of the Warchief's bodyguards flees, try branding the Warchief. Then no new bodyguard will take his place, and you can kill him later (leave him for last in case you need more points, giving you time, and you can deal with him easily with a downward arrow (on PS)).

Preparation:
First, enter the challenge and run around the map; it's important to know the locations of Graug baits.

Now start the trial again and begin the walkthrough.
The first thing you need to do is find intel on the Warchiefs. If there is none near your starting location, don't waste time running around the map; instead, fast travel to the next tower. Once you know their locations, check if there are any Graug baits nearby. Then, complete the mission to make the Warchief appear and go to the bait to summon a Graug.

When the Warchief is present along with his bodyguards, you should devour the bodyguards first, then the Warchief. Never do it the other way around! First, if you kill the Warchief first, everyone will scatter. Second, the task requires you to kill Captains with beasts, and when you kill a Warchief, one of the bodyguards takes his place, and it doesn't count!

Once the Warchiefs are dealt with, gather intel on the locations of Captains! Note that you don't need to gather intel on them! Just grab the first Uruk you see and find out where the Captain is currently located. Then, travel to the specified area (preferably where there are baits) and devour enough Captains to reach 10.

Once you complete the mission "Kill 10 Captains using beasts," you can deal with the rest as you please, but the faster, the better.

Methods of disposal:
  • Devour with a Graug.
One click and you can search for the next one.

  • Stealth kills.
These often either don't work or only take away half the health.

  • Bow.
If the Captain takes damage from the bow, it's best to use it (so you don't have to waste time killing regular Uruks). This method is good if you have runes that restore focus (time will be slowed) and arrows.

  • Sword.
One-on-one, this method is fast, but usually, nearby Uruks will try to stab you in the back with an axe.

***Looking forward to your results in the comments!***
DLC The Bright Lord

Purifying Mordor
Challenge the Dark Lord in Mordor.

Story achievement. Obtained at the beginning of the game.

Dominion
Break the will of 50 Uruks with a Wraith Flash.

Build up the red combo meter and brand Uruks with the Wraith Flash.

Burning Shadow
Brand 20 Uruks with Shadow Brand.

Aim your bow at an Uruk and brand him.

From Shadow to Shadow
Stealth brand a Captain.

Find a Captain you can stealth kill. Sneak up behind him and brand him.

Forges of Eregion
Build all Forge Towers in Udûn.

In the "The Bright Lord" DLC, there are 4 towers. Tower missions are similar to Warchief missions: complete a mission to make a Captain appear, then kill/brand him.

Battle-Hardened
Complete all The One Ring missions.

There are 4 missions of The One Ring in the game. Complete them to earn the achievement.

Silver Fist
Brand 20 Uruks in 60 seconds.

Cause a commotion in a fortress and gather many unbranded Uruks around you. Activate the ring and brand them with a Wraith Flash.

Betrayal
The One Ring has returned to its Maker.
Complete the final mission.
DLC Test of the Ring
In the game, go to "Trials of War" and select "Test of the Ring".

Epic Scope
Obtain a level 30 rune in the Test of the Ring.

1. 100% method for obtaining level 30-31 runes:
  • Find a Captain, brand him, and threaten a level 25 Warchief (it's important that he's near a fortress).
  • Die to the Warchief.
  • Brand the Warchief.
  • Summon the Warchief, create a commotion in the fortress, and pit the Warchief against unbranded Uruks.
  • Step aside and wait for your Warchief to be killed.
  • Pick up the level 30-31 rune.

2. Regular method:
  • Find a Warchief (+2)
  • Warchief's power should be 25 (+25)
  • Die to him (+1)
  • Gather intel on him (+1)
  • Exploit his fear or anger (+1).
  • Kill him
  • Pick up the level 30 rune - 25+2+1+1+1 = 30

Lord of the Ring
Complete all objectives in the Test of the Ring.

In this challenge, you need to:
  • Kill 5 Warchiefs and 12 Captains.
  • Brand 8 Captains/Warchiefs.
  • Brand 50 Uruks.
  • Complete the challenge in under 60 minutes.

Tips:

  • Ignore dropped runes! You can collect them by starting the trial again.
  • Stun Captains/Warchiefs to quickly build up the combo meter and perform a finishing move or execution.
  • Don't forget to use the ring.
  • When you appear, summon your Uruks and interrogate them about the Captains. Repeat until you have information on the whereabouts of all Captains.

Then, fast travel to the tower at the bottom of the map in the middle and look left towards the fortress in Wraith mode. See the Graug symbol there? If not, fast travel to this tower until a Graug appears.

Mount the Graug and, using the map as a guide, find and devour Captains. Once you've killed 12 Captains, dismount the Graug, brand the remaining Captains, and gather intel on the Warchiefs from them.

Kill the Warchiefs.

If you've branded all the remaining Captains, you can advance time, and the Captains will become bodyguards. This will make killing Warchiefs easier, but note that if your Captain dies, the branded Captain counter will decrease!
