The basics
At Noodles' stand, you can exchange one of your trinkets against one of Noodles' trinkets. You lose the stats from the trinket you give up, gain the stats from the trinket you gained (the amount of stats gained and lost depend on which week you're at, see table below).
You always have six trinkets, but only four are available at a time. Similarly, Noodles has a ton of trinkets but only has four available at a time.

@Leafpool10120 You mean in hard mode, even on week 4, the stats gained and lost from Noodles stayed at 2? Dang, will have to try that myself (and also easy mode, just to make sure). Thanks!

By the way, I played 4 weeks in hard mode and every week stats were increased/decreased by 3 (and the swap shop only increased/decreased by 2)

I have added a table in the first part telling you how many stats you gain/lose depending on which week you are. I have only checked up to week 4, so the later weeks might not be up to date if anything has changed, take them with a grain of salt.
