I played on the first save file at least 700hrs. Then I decided to complete the other 2. Is not that hard, if you plan beforehand what you're going to do, you can save a lot of time.
E.g you can get 7 completion marks if you are lucky (Boss Rush, Mom's heart, Hush, Isaac, ???, Mega Satan, Delirium).
Needless to say, practice is important.
