I still highly recommend this game to anyone that is a fan of action-rogue-lite. I can't say that any other rogue-like or rogue-lite I have played matches the replay-ability of this game. The developers are still updating the game and fixing issues.
While it may be difficult for many players, the feeling you get when you do win is great. There are many items and secrets to unlock, making the game seem to have no end.
I would advise you to look at the wiki associated with this game (or platinumgod.co.uk for info on item effects, after your first few runs) so that you can avoid some unnecessary deaths, but there is no harm in running into the game blind either, if you don't mind learning from experience the difficulty an anti-synergy can bring. 
If you are on a budget, I would recommend waiting for a sale before buying this, as getting every piece of the game will cost you a decent amount at full price.
In addition to this game being fun and challenging, the soundtrack is amazing. If you are into dark-themed music, you can even listen to it while not playing the game. The only reason I might not recommend this game to someone is if you are not fond of anything that may be perceived as slightly offensive. However, even if you do find some of the themes in the game offensive, it might still be worth the buy due to how much replayability there is to be had.
Lastly, if you enjoy a story that could potentially have multiple interpretations, then this game also has that, so be wary of browsing for information about it, as you may come across some spoilers.
